The Affordable Care Act (ACA) calls for the expansion of state Medicaid programs to increase access to health care coverage. Medicaid is a publicly-funded program that covers health care costs for lower-income Americans who qualify. Although the ACA creates incentives for all states to expand their Medicaid programs; not all have done so due to the U.S. Supreme Court’s ill-conceived 2012 decision allowing states to choose whether or not to expand. The choice not to expand has left a dangerous gap in coverage for millions of Americans, including many living with HIV. Given that the HIV epidemic disproportionately affects lower-income Americans, Medicaid expansion is vital to ensuring access to life-saving health care for thousands of Americans living with HIV and implementation is a necessary condition for the United States to begin the process of ending the HIV epidemic.
